The purpose of this questionnaire is to provide information that will assist the architect in development of a program of space needs for St. James’ Episcopal Church. Please fill out one form for each space or function that should be accommodated in the renovated facility. You are encouraged to provide any commentary or supplemental information you believe will be useful for this programming phase. Please attach additional pages to this form if required for comments/observations and keep a copy of the completed questionnaires for your records.
